What You'll Do
Evaluate and treat patients referred to the Physical Therapy department
Develop and implement individualized treatment plans based on clinical evaluations and patient goals
Educate patients and families regarding diagnoses, treatment plans, and home exercise programs
Monitor patient progress, modify treatment plans as needed, and develop discharge recommendations
Complete evaluations, progress notes, treatment documentation, and discharge summaries accurately and timely
Collaborate with physicians, nurses, case managers, and other interdisciplinary team members to optimize patient outcomes
Participate in care planning, case management discussions, and departmental meetings
Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and volunteers
Support departmental initiatives, quality improvement efforts, and student clinical education programs

Requirements
Bachelor’s degree (minimum) in Physical Therapy from an accredited program
Licensure & Certifications:
Licensed Physical Therapist in the State of Indiana
Current CPR certification
