Growing together.As the Home Health Patient Care Manager is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Primary Responsibilities:
- Oversees patient admissions and care coordination, including referral intake, clinician assignment, Start of Care (SOC) planning, insurance authorization, and verification of home health eligibility and medical necessity
- Manages and ensures the development, review, and ongoing updates of individualized patient plans of care in accordance with physician orders, regulatory requirements, and best practices
- Serves as the central clinical communication hub, coordinating with physicians, clinicians, patients, families, and interdisciplinary partners to manage orders, labs, changes in condition, and transitions of care
- Provides clinical oversight and quality assurance through documentation review, case conferences, utilization monitoring, risk reporting, and compliance with clinical, coding, and payer requirements
- Provides clinical leadership and support, including staff guidance, orientation, direct patient care as needed, participation in quality improvement initiatives, and adherence to on call and episode management processes

Required Qualifications:
- Current Physical Therapist or Occupational Therapist licensure in state of practice
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
Preferred Qualifications:
- Current CPR certification or ability to complete within 90 days of hire
Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $89,376 to $134,064 annually based on full-time employment. We comply with all minimum wage laws as applicable.
