Join our team of pediatric hematology and oncology pharmacists to care for our patients in the acute care and clinic spaces.  Sign on bonus available. 

Key Responsibilities:

• Deliver direct patient care through evidence-based pharmacotherapy management and medication optimization.
• Provide patient and caregiver education regarding medications, adverse effects, adherence, administration, and safe handling.
• Review, verify, and interpret medication orders for accuracy and appropriateness.
• Collaborate with healthcare providers to support formulary compliance and access to specialized medications.
• Lead and participate in medication safety, quality assurance, and performance improvement initiatives.
• Ensure compliance with all applicable policies, procedures, and regulatory requirements.
• Supervise pharmacy operations and support personnel to ensure safe, effective medication-use practices.
• Precept, mentor, and evaluate pharmacy learners and staff.
• Deliver age-appropriate pharmaceutical care across the continuum of care.

Requirements

Licensure, Registration, and/or Certification Required:

Licensed in the state(s) of practice or eligible for reciprocity required.
Immunization and non-vaccine injectable certification required based on area of practice.
Credentialing and privileging required based on area of practice.

Education Required:

Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
Completion of a PGY2 in Oncology OR Pediatrics.

Experience Required:

No experience required.
Will consider: Doctor of Pharmacy with at least 5 years of clinical pediatric experience who has obtained their BCOP OR BCPPS.
