An outstanding opportunity is available for an experienced RN Case Management leader to join a respected acute care hospital in the Rehoboth, New Mexico area. This is a high-impact leadership role for a clinically strong and operationally driven healthcare professional who thrives in a collaborative environment and is passionate about patient advocacy, care coordination, and quality outcomes.
This position offers the opportunity to lead a well-established Care Management department while partnering closely with hospital leadership, physicians, payers, and community resources to ensure patients receive the highest level of care in the most appropriate setting.
Position Overview
The Director of Case Management will provide both strategic and day-to-day operational leadership for the Care Management department and Swing Bed Program. This leader will oversee departmental operations, staff development, regulatory compliance, utilization management, and patient throughput initiatives while maintaining a strong focus on quality, efficiency, and patient-centered care.
Key Responsibilities
• Lead, mentor, and develop a multidisciplinary Case Management team while fostering a culture of accountability, collaboration, and excellence
• Oversee all Care Management operations including utilization review, discharge planning, care coordination, and regulatory compliance
• Direct and support the Swing Bed Program, including program development, education, community outreach, and compliance initiatives
• Serve as a patient advocate with strong working knowledge of medical necessity criteria, payer guidelines, and insurance authorization requirements
• Collaborate with physicians, hospital leadership, insurance providers, and community agencies to optimize patient outcomes and resource utilization
• Monitor departmental metrics, reporting, and performance improvement initiatives
• Ensure compliance with CMS Conditions of Participation, accreditation standards, and recovery audit preparedness
• Assist with case management responsibilities as needed while leading by example and supporting best practices across the department
Qualifications
• Current RN license required
• BSN strongly preferred
• Minimum of 5 years of Case Management experience within an acute care hospital and/or managed care environment
• Prior leadership experience within Case Management strongly preferred
• Strong knowledge of utilization management, discharge planning, payer regulations, and accreditation standards
• Proven ability to lead teams, manage departmental operations, and drive process improvement initiatives

Requirements
Current RN license required
• BSN strongly preferred
